Samenvatting
Gerandomiseerde neoadjuvante fase 3 studie met 3 armen voor patiënten met spierinvasief urotheelcelcarcinoom van de blaas die niet in aanmerking komen voor cisplatin.
Arm A: Durvalumab + tremelimumab + enfortumab-vedotin, gevolgd door cystectomie, en adjuvant durvalumab + tremelimumab
Arm B: durvalumab + enfortumab-vedotin, gevolgd door cystectomie, en adjuyvant durvalumab
Arm C: standaard behandeling met cystectomie zonder adjuvante behandeling
De studie begint met een safety run-in (n=20), waarin alle geïncludeerde patiënten worden behandeld conform Arm A.
Inclusiecriteria (belangrijkste)
Spierinvasief urotheelcelcarcinoom v.d. blaas cT2-4aN0-1M0
- Fit bevonden voor cystectomie
- ECOG 0-2
- Niet geschikt voor standaard neoadjuvante therapie middels cisplatin/gemcitabine ivm:
- Creatinineklaring <60
- ctcAE gr ≥2 gehoorsverlies
- NYHA classe III hartfalen
- ECOG PS 2
- Patiënt weigert neoadjuvant platinum-bevattende chemotherapie
- Check voor volledige inclusie studieprotocol
Exclusiecriteria (belangrijkste)
- Aanwezigheid van N2+ of M1 ziekte of T4b
- Noodzaak tot nefroureterectomie of ureterectomie
- VG met orgaantransplantatie wv immuunsuppressie, of allogene stamceltransplantatie
- Auto-immuunziekte (met exclusie van o.a. vitiligo, hypothyreoidie, chronische huidziekte wv geen systeemtherapie)
- Graad 2 neuropathie
- Baseline HbA1c ≥8
